dc.description.abstract | This audit showed that not all admission procedures involving (MR) using the Orion and Kardex systems were performed to a a satisfactory level. There is a huge gap between the current practice of (MR) and the set guidelines. The exception is that most patients have a pharmacist review within 24 hours, which is close to the standard set for this audit. In addition, there exists a lack of training for prescribers and pharmacists. | |
