Abstract

From the results of this research, it can be concluded that the FMEA process highlighted potential errors in our HDR brachytherapy emergency procedure. We were able to define FMs that could potentially and seriously affect patient treatment and staff exposure to radiation. It also showed that many high-scored FMs mostly related to human errors could be reduced by establishing procedural guidelines, appropriate personnel training of emergency procedures and development of QA. For staff involved in an emergency procedure, it should be noted that they should be fast in responding to an emergency, because the longer time they take to terminate emergency the higher the dose they and patient will receive.
