Carbapenem-resistant A. baumannii is a high-priority pathogen due to its resistance to multiple antibiotics, often requiring "salvage therapy." study aimed to study Molecular characterization and functional of Resistant carbapenem (Acinetobacter baumannii) from different Hospital at southwestern Region – Saudia Arabia. From December 2024 to February 2025, 50 clinical Acinetobacter baumannii isolates were obtained from hospitals in the Jazan region of Southwest - Saudi Arabia. Isolates were cultured, purified, and identified using the VITEK 2 system. Antimicrobial susceptibility testing was conducted against various antibiotics. Vesicles were isolated, and genomic DNA was extracted. Antibiotic resistance genotyping was analyzed using conventional PCR targeting 9 genes. Result of this study examined 50 Acinetobacter baumannii isolates from hospitals in Southwest Saudi Arabia, primarily from patients aged 51-90 years, males, and Saudi nationals. Antimicrobial susceptibility testing revealed high resistance to most antibiotics, with resistance genes OXA-23, OXA-51, and ISAbal found in 90% of isolates. Further investigation of OMVs from four samples identified OXA-51 and ISbA1 genes in three samples, but OXA23 and NDM genes were not found in the OMVs of these samples. Acinetobacter baumannii is prevalent in clinical settings in Saudi Arabia, particularly in older patients and intensive care units. High resistance rates of antibiotics like carbapenems, cephalosporins, and fluoroquinolones are alarming, limiting treatment options. Molecular research reveals resistance genes like blaOXA-23, blaOXA-51, and ISAba1 in isolates. Some outer membrane vesicle samples also contain resistance genes, suggesting resistance genes may be transferred horizontally.
